Vantage North Humber Teacher Training is committed to ensuring a safe, respectful and supportive environment for all trainees, staff and partners. This form allows trainees, mentors, staff or other stakeholders to raise a concern about any issue relating to:

• Well-being
• Safeguarding or safety
• Professional conduct
• Training provision
• Behaviour, relationships or support

Concerns may relate to yourself, another person, or the wider training environment.

All concerns will be taken seriously and handled sensitively, confidentially and in line with Vantage North Humber policies and procedures. Your anonymity will be protected as far as possible, if you make clear that is important.

IMPORTANT!

• If you believe someone is at immediate risk of harm, please contact emergency services or the Designated Safeguarding Lead immediately before completing this form.
• If the concern relates in any way to the safeguarding of pupil(s), please contact the school DSL and follow the school’s safeguarding procedure NOT this form, unless it is related to an allegation or incident involving a Vantage trainee or anyone connected with Vantage.
